Medical
Netezza offers the preferred provider organization (PPO) plan
through Blue Cross Blue Shield. This plan allows the participant
the option of seeing a provider either in or out of network.
Most in-network services are covered at 100% except for office
visit co-pay and a co-payment for inpatient and day surgery.
If a non-preferred (out of network) provider is chosen, a
deductible or co-insurance may apply. Key advantages of a
PPO plan are that no referrals are needed nor is there a requirement
for choosing a primary care physician.
Dental
Netezza offers the Premier USA plan through Delta Dental of
Massachusetts. This plan has a network of over 100,000 dentists
nationwide and allows the participant the option of choosing
Delta participating dentists or out of network dentists. This
plan offers a broad range of preventative, restorative and
orthodontic services.
401K Plan
This plan is available for employees to contribute 1-100%
of basic annual salary on a pre-tax basis into a retirement
account up to the IRS annual maximum limit. There is no waiting
period, allowing employees to join immediately.
Paid Time Off
Netezza offers 10-11 paid holidays per calendar year, 3 weeks
of paid vacation accrued per year and 1 week of sick pay,
if needed. The total PTO package is up to 6 weeks annually!
Pre-tax Spending Account
Netezza's Flexible Spending Account allows employees to contribute
annually on a pre-tax basis for medical reimbursements up
to a maximum level.
Life Insurance
Employer paid life insurance covers 2x basic annual earnings
to a maximum amount.
Disability
Short Term Disability is employer paid, 100% coverage for
first 8 weeks and 66 2/3% coverage for remaining 4 weeks.
Long Term Disability is employer paid, 60% of monthly income
after 90 days of disability to a maximum monthly amount.
Employee Assistance Program
Netezza provides an employee assistance program that offers
free, confidential counseling services as well as general
information services to help employees and their family members.
